Group Personal Accident Insurance Claims

1. Incident/Accident report

2. Affidavit of Driver and Photocopy of Driver’s License (if Vehicle Accident)

3. Police report

4. Medical Certificate - to determine nature and extent of injuries and duration (period of incapacity)

5. Medical/hospital receipts, invoices or other evidences of expenses.

6. Death certificate and funeral certificate of post mortem examination (autopsy report) – to determine the cause of death.

7. Birth Certificate/Marriage Contract/other proof of paternity (guardianship proceedings) to determine the proper payee.

8.Affidavit of Desistance/Release of Claim.
